This one-page document gives you your "R.D number". Sometimes, this document is enough for insurance purposes. Often, it isn't and you need to obtain the full report.
Obtaining the full report can be done in person. But you have to travel to the Central Police Headquarters during their shortened business hours, park, wait in line, fill out multiple forms, wait in more lines, and more. Most people don't have time for this.

After we submit the request, the City of Chicago will pull the records and send them to you by mail. This usually takes 10-14 business days to obtain to your mailbox.

The RD (or R.D.) number can be found at the top right of your Victim Information Notice from the Chicago Police Department. Here is a sample with the location of the RD number circled in red.
